Documents you may notice when when registering for City of Emeryville Youth Programing...
All youth under the age of 18 are required to have a concussion protocol form on file with the department. The form is valid for one (1) year. It is important for all families to know and understand our concussion protocol as it may affect your child's ability to participate in programing after sustaining a head injury. General and Covid-19 Liability Forms
All program participants must sign the City's General Liability and Covid-19 Liability forms prior to registration. Please read them all thoroughly prior to registering and let us know if you have any questions.
